Taekwon-Do and martial arts training involve physical activity and, as with any sport or recreational activity, there is a risk of injury. While Baekho Taekwon-Do takes reasonable care to provide a safe training environment, participation is undertaken at the student’s own risk.

By signing this form, the student, or parent / guardian where applicable, acknowledges and agrees that:

I understand that participation in Taekwon-Do involves physical contact, exertion, and the risk of injury. I declare that the information provided in this form is true and correct to the best of my knowledge. I will notify Baekho Taekwon-Do of any change to medical or emergency contact information. I understand that students should follow instructions given by the instructor and behave respectfully and safely during training. I acknowledge that training fees contribute to club operating costs including hall hire and insurance. I consent to participate, or give permission for my child to participate, in training conducted by Baekho Taekwon-Do.
